Application and Fees   To promote an atmosphere of small group interactions, attendance at FOCAPD '99 will be limited and is by invitation based on this application. Applications must be postmarked by January 15, 1999. Invitations, which will contain detailed registration and payment information, will be sent as soon as your application is accepted, but no later than February 15, 1999. The conference fee includes registration, proceedings, welcoming reception, hospitality and coffee breaks, and the conference banquet. The conference fee is: · $675 for registrations paid on or before March 15, 1999. · $750 for registrations paid between March 16, 1999 and May 14, 1999. Registrations and fees will not be accepted after June 1, 1999. Please note that there is a $75 administrative fee for all cancellations.
